Two Launches, One Coverage Decision, and a $1 Billion Target
An eye-care company just put a $1 billion sales target on 2027. The Medicare decision is the test.
An eye-care company just told investors its two new launches can carry sales to $1 billion next year, even if Medicare tightens coverage. You get a real growth story, but after the stock jumped sharply on the report, the entry price matters more than ever.
Yes, that Arnold Schwarzenegger.
He has a newsletter on beehiiv. So do Codie Sanchez, Jay Shetty, David Begnaud, Colin & Samir, and Joanna Stern.
They could publish just about anywhere. They chose beehiiv to build a direct connection with the people who want to hear from them most. And you can do the same, whether you’re world-famous or just at the start of your plan for world domination.
Glaukos Corporation

November 2 – Pre‑market
Ticker: GKOS | Sector: Healthcare (Medical Devices and Ophthalmic Pharmaceuticals) | Market Cap: $14.07B
30‑Second Take
Glaukos introduced preliminary 2027 net sales guidance of $1 billion alongside its third-quarter report on October 28. Management said that outlook already factors in downside scenarios for Medicare coverage of iDose TR, its implant that releases glaucoma medication inside the eye.
The second engine is Epioxa, the first and only FDA-approved epithelium-on corneal cross-linking therapy for keratoconus, a rare disease that thins and bulges the cornea. Epioxa contributed $53 million in its first full quarter with a product-specific J-code, the billing code that makes reimbursement routine.
This is a launch-and-coverage idea, not an earnings-gap trade. Your upside depends on iDose TR keeping its Medicare access and Epioxa reaching more patients, while your main risk is paying a stretched price for a stock that jumped 33% right after the report.
The shares trade near the top of their 52-week range after that move. You are buying strength, so the plan should focus on waiting for the stock to digest that move rather than chasing it.
Trade Setup
Time frame: Medium-term, three to six months
Edge type: Product launches and a Medicare coverage catalyst
Entry Zone: A pullback toward the $210 to $220 area, where the post-report session opened, rather than a chase near the highs. Upside Target: $255 to $270, the upper end of the analyst price targets raised after the report. Stop: A close below $200, which would erase most of the post-report gain.
Watch for the final Medicare local coverage determination on iDose TR. A coverage policy that preserves physician choice would remove the biggest overhang, while a restrictive one would test how much of the $1 billion target truly rests on Epioxa.

Chart

1-Month Trading Summary: GKOS climbed sharply in October, with most of the gain coming right after the report, when shares jumped significantly on several times normal volume. The stock had already been climbing into the print, and it trades near its 52-week high.
That kind of gap often gets partly retraced as short-term traders take profits. You should treat a calm, low-volume pullback that holds above the gap area as healthier than a fresh spike.
Bull Case
Why It Works
Glaukos is turning from a device company into an ophthalmic pharmaceutical company. Non-GAAP gross margin reached 88% in the third quarter, reflecting a shift toward higher-margin drug products such as iDose TR and Epioxa.
iDose TR generated $76 million in the quarter, helped by growing use among traditional Medicare and Medicare Advantage patients. A recent Phase IV trial showed an 11.1 mmHg mean pressure reduction and statistically significant superiority when combined with cataract surgery versus cataract surgery alone, giving surgeons more evidence to support adoption.
The Catalysts
Epioxa's rollout is still early. Glaukos says its treatment sites already serve roughly 90% of the U.S. population, with more locations working through approvals that would lift that reach above 95%, and access pathways cover 90% of U.S. commercial lives.
The company also plans to launch a keratoconus screening device as it enters 2027 to help optometrists find undiagnosed patients. Further out, it is targeting FDA approval of iDose TRIO by the end of 2027, advancing the next-generation iDose TREX, and expanding into retinal disease through its RevOpsis agreement for the biologic GLK-901.
What The Numbers Say
Third-quarter net sales were a record $211.0 million, up 58% from a year earlier, and management raised its 2026 sales guidance to $775 million to $785 million from $680 million to $700 million. Corneal health sales rose 154% to $59.0 million, and U.S. glaucoma sales rose 47% to $118.6 million.
The balance sheet is moving the right way, too. Cash, short-term investments, and restricted cash rose by $50.5 million during the quarter to $339.8 million, so the launches are not draining the company's reserves.
Bear Case
What Could Break It
Medicare contractors proposed local coverage determinations for iDose TR in May, and management acknowledged that a restrictive final policy could limit reimbursement and physician adoption. The contractors have up to a year from the draft to finalize or abandon it, and a final policy would take effect after a 45-day waiting period.
Epioxa is a new product that depends on prior authorizations, specialty pharmacy workflows, and earlier diagnosis of an underdiagnosed disease. Any slowdown in that ramp would hit sentiment quickly because so much of the 2027 target depends on it.
The Valuation Risk
At $14.3 billion, the market values Glaukos at more than 14 times its preliminary 2027 sales target. The company is not yet profitable on a trailing GAAP basis, and third-quarter GAAP net income was just $2.0 million.
Spending is rising as well, with full-year 2026 operating expenses expected at $630 million, more than 30% higher than last year. If sales growth slows while that investment continues, the premium multiple has a lot of room to compress.
How To Size It
Start smaller than usual. You are buying a stock that just made a large one-day move into a pending regulatory decision, so a single headline could send it sharply in either direction.
Add only if the shares hold the post-report gap and the coverage outcome is clear. If the final Medicare policy comes in restrictive, reassess rather than average down.
Quick Checklist
✅ $1 billion 2027 target is preliminary and includes downside iDose TR coverage scenarios
✅ Final Medicare coverage policy for iDose TR is still pending, with no date set
✅ The shares trade near their 52-week high after a sharp post-report jump
